The word sam covers three ideas: being alone (był sam), doing something oneself (sam to zrobił), and pointing to the very thing (w samym centrum). It agrees with its noun in gender, number and case, so its endings change: sam, sama, samo, sami, same, samego, samym…
How it works
sam agrees with the noun it refers to. Alone: był sam, była sama, były same. Emphatic '-self', placed next to the subject: sam to zrobił = 'he did it himself'. 'The very', right before the noun: w samym centrum = 'in the very centre', do samego rana = 'until the very morning'. In oblique cases it takes adjective-like endings: samego, samemu, samym, samych.

Good to know
Do not confuse sam with the reflexive się. sam to zrobił means 'he did it himself'; zrobił to sobie means 'he did it to himself'. Also keep sam separate from the intensifier sam siebie ('oneself' as object): widział samego siebie w lustrze = 'he saw himself in the mirror'. Placement matters: sam centrum is wrong — the fixed order is w samym centrum.
